You have been invited to take part in a military charities sector survey. The purpose of the survey is to help us understand how military charities function to inclusively support a diversifying military community. Taking part in the survey is completely voluntary.

Consent to Participate

I have read the Participant Information Sheet and have had the opportunity to consider the information, ask questions and have had these answered satisfactorily.

I understand that your participation is voluntary and that you are free to stop or withdraw at any time during the survey without giving any reason, without being disadvantaged in any way.

I understand that the data will be stored on the Halsden Centre’s OneDrive in password-protected files for a period of three years after the completion of the survey.

I understand that individuals from the Halsden Centre may look at data collected during the study to ensure that the study is conducted appropriately. You give permission for these individuals to have access to your research data.

I understand that the information collected will be used to support other research in the future and may be shared anonymously with other researchers.

I consent to direct quotes being used in the write up of the research or other outputs related to the programme, including conference presentations, marketing materials, reports and journal papers. You understand the quotes will not be attributed to you and will be anonymised.
